What is a painless birth?

by clinic

Painless delivery; It is a completely normal birth in which labor pains are perceived as pressure in the abdomen and pressure in the groin. The only difference is that you feel your labor pains as just contractions in your belly (and pressure in your groin as the baby comes out). There is no pain. After the regional analgesia application, you can walk and walk around in your room during the period until the birth. When you take you to the delivery room and position you, when you feel a contraction in your abdomen and pressure in your groin, you push your baby down simultaneously. With each contraction, the baby moves a little further down the birth canal, and finally the head and then the body are born.

If you wish, let’s first examine the normal birth and its stages, let’s see how the pains occur:

Birth and its Stages
Birth, the baby in the mother’s womb progresses through the birth canal and comes out with various contractions when the time comes, is to open your eyes. Every woman’s birth is special and an unforgettable experience for her.
Normal birth takes place in three stages (stages):
1-Opening (Dilatation)
2-Delivery of the baby (Episiotomy is opened beforehand if necessary.)
3-Delivery by Halas (exit of placenta-baby’s spouse) action is completed.

Birth time:
Birth time; first or 2nd-3rd. Although it can change with the birth, the height and weight of the baby and the mother, the position of the child, the anatomical structure of the canal, the mental state of the mother and many similar factors; an average of 6-9 hours at first birth; in subsequent births, it is 3-5 hours.

Birth Pain
Labor pain is a multifaceted pain with its own characteristics and is usually very severe. For many women, it is the most severe pain they have ever experienced. Pain in the first stage of labor is caused by uterine contractions and dilation of the cervix, opening the birth canal. These rhythmic contractions are felt in various forms and are like severe menstrual pain. It is caused by the contraction and stretching of the soft tissues at the base of the groin and hip as the baby’s head goes down and out of the birth canal.

Some methods such as hypnosis, acupuncture, Aromatherapy, Etonox gas, TENS application and Pethidine injection are tried in the first stage of labor. However, all may not be sufficient later on. Only Regional Analgesia methods are effective in the later stages of labor.

Analgesia means “no pain, no pain, no pain”. Regional analgesia is the temporary numbing of only the area where the pain occurs or transmitted, without putting the person to sleep (while he is conscious), and eliminating the pain and/or sensation in that area for a while.

Regional analgesia can be applied with 3 different techniques in normal delivery. The most common method is epidural analgesia. The second method is spinal analgesia. The aim here is to benefit from the advantages of both methods.

Gold standard in normal delivery: Epidural Analgesia.

It is the most preferred, most effective, safest and most frequently used method for normal delivery. It is performed by injecting pain relievers around the dura mater membrane (Epidural area) in the lumbar region. The expectant mother feels the pressure, the touch, she can even get up and walk, but she does not feel the pain. There are labor pains and contractions necessary for normal birth, but they do not bother.

When is it applied?
It is applied when the uterine contractions become regular (birth pains settle) and the cervix thins by about 60-70% and its opening reaches 4 cm, that is, when the pains start to seriously disturb the mother. Application before it affects the contractions and delays the birth. If it is late, both the mother will suffer unnecessary pain and the pain will come more frequently, so the expectant mother cannot remain still during the procedure and epidural administration may become difficult.

EPIDURAL ANALGESIA AT BIRTH

* Today, it is possible to eliminate labor pains without numbness or feeling of heaviness in the feet, in other words, “Mobile painless birth” is possible. You can walk after the application.
* Epidural analgesia doesn’t make you dizzy, doesn’t make you feel sick, and doesn’t stop bowel movements or prevent flatulence.
* Increases your chances of normal delivery by both psychological relief and resolution of muscle spasm.
* It relieves most of your birth stress, provides relaxation and relaxation in the muscles.
* You feel the pain not as pain, but as pressure and contraction. You can participate in the birth by pushing the contractions simultaneously during the ascent, you can speed up your birth.
* Since you are not tired or exhausted after birth, you can hold your baby in your arms much more quickly and feed it immediately.
* Post epidural headache performed by experienced specialists is very rare (around 1%). Even if it does, it can be treated. Even if it is not treated, it goes away on its own within 7-15 days, without leaving a trace.
* The gold standard for pain relief in normal labor is “epidural”.
