Roux-en-y gastric bypass surgery is the most frequently performed and proven effective obesity surgery worldwide.
In this surgery, first a small part of the stomach is cut from the other part and separated. Then, the intestinal segment is cut and separated 50 centimeters from the beginning of the small intestines. The end coming from the stomach side is connected to an intestinal part 150 cm ahead, and the other end is sewn to the small stomach part formed. Thus, the bypass process, which allows the food to reach further by bypassing a part of the stomach and intestinal segments, is realized.
Although it is a method that has been used for years with proven effectiveness, its use has started to decrease gradually due to its many disadvantages and the increasing prevalence of sleeve gastrectomy surgery.
The disadvantages that push gastric bypass surgery into the background are that the surgery is more difficult and takes longer, and therefore the risk is higher. In the postoperative period, mineral and vitamin deficiencies are more common, ulcers may occur in the intestines. Since the small pouch formed in the beginning of the stomach and the other parts are disconnected, it becomes impossible to reach the parts other than the small pouch with the endoscope, and therefore, endoscopic diagnosis and treatment methods cannot be used for these parts. In this surgery, since the passage of food is changed, that is, the foods that normally need to pass from the stomach to the 12 finger intestine and from there to the other small intestine segments bypass the last part of the stomach and the 12 finger intestines and reach the small intestines directly. In some cases, a situation called dumping syndrome, which causes complaints such as nausea, vomiting, diarrhea, weakness, occurs due to the sudden passage of concentrated foods, especially sweet foods, into the small intestines.
Despite all these, there are still many surgeons who use it as the first choice for bariatric surgery. In addition, in some cases, we prefer it out of necessity. If a patient has reflux or has had unsuccessful sleeve gastrectomy before, we may need gassic bypass surgery. Glastic bypass surgery is the first choice in the treatment of diabetes patients using insulin.
