If the appropriate treatment is not started early, acne can lead to spots and scars on the skin. The spots that develop after acne can be light or dark in color from the skin, or they can be red in color. Although light or dark spots from the skin usually regress spontaneously within weeks or months, some medications, superficial peeling procedures (chemical peeling, Spectra peel, fractional laser) and strict sun protection accelerate the healing process. In the treatment of red spots, laser and IPL devices that are effective on vascular structures can be used in addition to the drugs to be applied.
The scars that develop after acne may be raised from the skin or sunken from the skin. Raised skin scars are usually resistant to treatment and may recur even after successful treatment. In the treatment of these scars, cryotherapy with liquid nitrogen and injection of drugs into the skin are often used.
Depressed scars from the skin level should be defined correctly as treatment methods differ. First of all, TCA CROSS application is the simplest but most effective method in the treatment of scars with a narrow mouth, thin but extending to deep tissues (icepick). In this method, TCA in such a high concentration (50-100%) that it is not suitable for application on the entire face is applied only into the scars, thereby reducing the depth of the scars. In addition to the treatment of these scars, minor surgical interventions (punch excision, punch elevation, punch grafting) with a medical device that can make round incisions in the form of punch holes are also very effective.
Secondly, let’s talk about the tracks with a wide base, separated by a steep slope from the skin level (boxcar/wagon). In the treatment of these scars, in addition to the treatment methods mentioned above, fractional laser and radiofrequency applications and laser full-thickness peeling can be preferred.
Traces in the form of wide depressions (rolling/slope) separated from the skin level by a slight inclination are best observed with angled light. The base of these scars is adhered to the deep tissues by bands of connective tissue. In the treatment of these scars, special needles must be inserted under the skin and these connective tissue bands must be broken and the base of the scar should be freed (subcision). In addition, the above-mentioned treatment methods can also be applied.
In the same patient, more than one of these scars and spots are often found together. Therefore, an individualized treatment plan is needed for each patient. As mentioned above, there is no miraculous device or method that is effective for all of these post-acne conditions.
