The aim of surgery in the treatment of spinal metastases is to achieve maximum functional clinical well-being by using the least invasive method.

If the primary focus is unknown and there is no pathological diagnosis: If the lesion is easily accessible, tissue diagnosis should be obtained by needle biopsy first. Because there is a possibility of epidural abscess in cancer patients.

If spinal column is unstable

If neurologic deficit is due to spinal deformity or bone compression rather than tumor compression

If XRT fails

If patient’s expected life expectancy If it is more than 3-4 months

Surgical treatment should be planned according to the patient’s request
