With the use of laser technology in the last 15 years, significant progress has been made in the treatment of acne scars, pores, scar and burn scars, and facial and neck wrinkles.
In recent years, 3 types of laser systems have been designed for effective skin rejuvenation and scar treatment; Today, these systems are preferred as the most successful methods in the treatment of various scars and wrinkles:
1. Erbium:Glass laser (Er:Glass): 1,540 nanometer wavelength
2. Erbium:YAG laser (Er:YAG): 2,940 nanometer wavelength
3. Carbon dioxide laser (CO2): 10,650 nanometer wavelength
The most important factors that determine the efficiency of the laser are; The wavelength of the laser beam, the technological features of the laser and the skin structure of the patient.
Fractional Er:YAG Laser treatment has been applied in our clinic for about 4 years. Fractional MCL 30 Laser is a new generation laser technology. It can be applied for skin renewal, tightening the collagen structure, easing wrinkles around the eyes, treating skin blemishes and scars without creating scars on the skin. It creates thousands of small but deep bridges, known as “microthermal treatment” areas, within the intact tissue of the skin and creates microscopic small wounds. This “very small-microthermal treatment” allows the skin to heal much faster than treating the entire area as a whole. With the fractional laser method, it is aimed to create a new, healthy, tighter tissue and to eliminate skin defects by using the natural healing process of the skin. Thus, the skin content is regulated, spots are erased or reduced; skin color improves, quality and elasticity increase.
The thickness of the treated skin during fractional laser applications is not dependent on judgment or experience, as in chemical peeling and dermabrasion; depth is determined. Although its effect is strong, the recovery time after application is short thanks to its fractional feature. The expected result of the treatment is much more positive than similar systems.
Usage areas of Fractional Laser:
1- Treatment of acne scars: The most effective method for removing acne scars is fractional laser. As a result of scanning the area with a special light, the scar tissue is evaporated. The evaporated portion is filled by the body with fresh cells. The scar tissue gradually gets smaller with each session. An average of 3-6 sessions are applied. After the session, redness and crusting occur.
2- Treatment of pregnancy stretch marks: The laser affects the tissue in a deeply porous way. Evaporates uneven areas. Fresh tissue replaces worn tissue. Fine cracks are erased, deep cracks are lightened.
3- Scar treatment: Thin scars are erased. It is used in the treatment of surgical scars, impact and razor wounds. Light beams sent to the scar area evaporate and fix the problem area.
4- Neck and décolleté rejuvenation: Worn and sagging neck and décolleté skin is renewed. Application with mesolifting or PRP treatment increases the effectiveness.
5- Removal of skin spots: Sun spots on the skin, birthmarks called melasma, age-related spots and changes in skin tone, acne spots, etc. It is a method used for treatment. More Erbium Fractional laser is used.
6- Treatment of bruises and bags under the eyes: Under-eye bruises make the person look more tired and older, this situation is called hyperpigmentation (increase in color) in medical language. It is usually observed more intensely in the inner part of the lower lid. Among the reasons; genetic predisposition, allergies, eye rubbing habits, some diseases (kidney, thyroid). Vitamin K creams and fade creams have been tried most for this color boost; however, satisfactory results were often not obtained. Pigmentation is reduced by evaporating the color substance under the eyes and restructuring the skin. The form of laser application is selected according to the skin characteristics of the patient.
In the light of clinical studies, 3-5 sessions are recommended at approximately 2-4 week intervals for an effective treatment and a good result. Although the effectiveness is seen immediately after the laser application, since the collagen remodeling takes 2-3 months, the real developments and results appear after 2-3 months. This gives us the opportunity to determine the appropriate fractional laser method according to the patient’s complaint and the characteristics of the skin structure, and to use different options in the planning of the treatment.
