Abstract Objective: This study evaluated the transradial approach for its ability to diagnose coronary artery anomalies, its requirement for catheter usage, the number of images obtained and fluoroscopy time required. Patients and methods: A total of 11,707 patients’ coronary angiograph reports from January 2009 to January 2016 were evaluated with 179 patients identified as having coronary artery anomalies. Subsequent analyzes compared patients’ access sites with multiple angiographic parameters, including the number of images obtained, catheters used, and the fluoroscopy time required. RESULTSesults: The frequency of coronary artery anomalies identified by angiographies was 0.015%. Coronary anomalies were detected by transradial access (TRA) in 133 patients and by transfemoral access (TFA) in 46 patients. by clinicby clinicWe studied 92 patients to determine the relationship between the QRS treadmill score (QTS) and extent of coronary artery disease. The results were compared to those of ST segment deviation in a < (+)QRS score > population. A cut off point of < 5 mm was used for mild to moderate risk patients whereas a QTS of < 3 was considered high risk for the patients. 43% of the patients had high risk QTS and 57% mild to moderate QTS. A highly significant correlation was observed between the extent of coronary artery disease and the high risk group of patients, whereas nonsignificant changes were observed in the control group (p < 0.001). Finally, the results indicate that in patients with a (+) QTS score (< 5 mm), coronary artery disease was significant and accordingly extensive during coronary angiography. On the other hand, a (+) QRS score proved to be superior to ST deviation in predicting critical coronary stenoses in the same population (F ratio; 41 vs 24).

The protective effect of topical rifamycin against sternal wound infection in diabetic patients undergoing on-pump coronary artery bypass graft surgery
by clinicby clinicSummary Objectives The aim of this study was to investigate the protective effect of topical rifamycin SV treatment against sternal infection (SWI) in diabetic patients undergoing on-pump coronary artery bypass graft (CABG) surgery. Methods One hundred and fifty-nine diabetic patients who were scheduled to undergo isolated on-pump CABG surgery were included. Eight were excluded for various reasons. Of the 151 patients, 51 were on insulin therapy and 100 were on oral anti-diabetics. The risk of mediastinitis was assessed using the American College of Cardiology/American Heart Association 2004 guideline update for CABG surgery. According to the risk scores, patients were divided into two comparable groups: the rifamycin group (n = 78) received topical rifamycin treatment after on-pump CABG surgery, and the control group (n = 73) received no topical treatment.

Epicardial fat tissue predicts increased long-term major adverse cardiac event in patients with moderate cardiovascular risk
by clinicby clinicWe investigated the relationship between epicardial fat volume (EFV) measured by multislice computed tomography (MDCT) and long-term major adverse cardiac events (MACEs). Consecutive patients (n = 564) were enrolled in this retrospective study. Patients were divided into tertiles according to EFV. Patients were followed up for an average of 18 months. Patients in each tertile were similar in terms of gender and risk factors. Patients with greater EFV in the third group were more likely to be overweight (P = .001) and older (P = .001). High-density lipoprotein cholesterol levels were relatively lower in the third tertile (45 ± 9, 45 ± 11, and 43 ± 9 mg/dL, respectively; P = .018). The third group had a significantly higher rate of myocardial infarction (0.6%, 1.1%, and 3.7%, respectively; P = .043).
