Case: 55-year-old male patient. Our patient has had low back, left hip and left leg pain for 10 years. There is disc herniation on the left L4-5, L5-S1 in MRI. VAS is 9-10. Our patient had DM for 15 years. Due to these pain complaints, he applied to many clinics but could not get any results. Then he applied to our clinic. We first started our patient with 75mgx2 pregabalin and asked for their routine biochemistry.
15 days later, the VAS was 7-8 in the control. FB was 150 and HbA1c was 8.58. We decided to perform an interventional block for our patient. First, we performed a caudal epidural block for our patient.
We told her to continue pregabalin 75mgX2 in her treatment, and her VAS was 5-6 at the follow-up 15 days later. Thereupon, we added a lumbar paravertebral block next to the caudal epidural block and increased the peregbalin to 150mgX2. After 15 days, the VAS was 3-4 in the control.
Thereupon, we made the same blocks again for our patient and planned the treatment exactly. We asked for his routine biochemistry and we called him for control 15 days later. When he came to the control, he was VAS 1-2. FPS 134 HbA1c had decreased to 6.87.
Conclusion: Intervention blocks have a very important role in the pain treatment of patients with lumbar hernia. Although our patient was diabetic and continued without changing the diabetic treatment, we see that his FBC and HbA1c also regressed.
