Role of interventional blocks in the treatment of diabetic foot neuropathic pain and wound

Case: 60-year-old male patient. Our patient applied to us with the complaints of burning, tingling and cramping in his feet. It was VAS8-9. This patient also had a non-healing diabetic wound due to amputation of the right big toe. For this, our patient was started on pregabalin 150mg/day. The complaints of this patient had decreased. Downgraded to VAS6-7. With the decrease of the effect of this drug, his complaints increase again and he rises to VAS8-9 again. Here, the dose of pregabalin was increased to 300mg/day and interventional blocks were planned. First, our patient underwent a caudal epidural block. He was VAS6-7 at control 15 days later. Meanwhile, in our patient’s right big toe amputation, it was observed that the non-healing wound started to heal. Thereupon, the right popliteal block was applied to our patient in addition to the caudal epidural block again. After 15 days, it was observed that VAS decreased to 3-4 and wound healing increased in the control. Caudal epidural block + right popliteal block was applied to our patient again. When he came to the control after 15 days, it was VAS0-1. In the meantime, it was observed that the healing of the foot injury continued. Thereupon, medical treatment was arranged for our patient, and he was called for follow-up with one month intervals.

When he came to these controls, it was seen that he had no complaints and his VAS was 0-1. In these controls, it was observed that the right foot wound continued to heal.

Conclusion: Here, it was seen that interventional blocks have a place in addition to medical treatment in diabetic neuropathic pain. It was also observed that these interventional blocks accelerated the healing of non-healing diabetic foot wounds. Therefore, we believe that interventional blocks can be used in non-healing diabetic foot wounds and diabetic neuropathic pain.
