FOAM THERAPY HISTORY OF SCLEROTHERAPY

The development of sclerotherapy, needle therapy or foam therapy, which is one of the methods used in the treatment of varicose veins, took place in the 20th century.

  • Tavel (1904) explained for the first time that if thrombosis does not occur despite the ligation of the varicose vein, thrombosis should be achieved by administering a substance.
  • Schiassi (1907) performed the first sclerotherapy, with the simultaneous injection of iodine solution with the ligation of the varicose vein. He did all of this with cocaine-based local anesthetic, which was popular at that time.
  • After 1925, both ligation and ligation became standard treatment.
  • Nobili (1921) suggested that Schiassi’s student should be given a sclerosing agent while connecting the VSM in the groin without the need for further incisions.
  • 1927 Unger demonstrates that injection can be made into the distal VSM with the aid of a catheter (the only urinary catheter urinary catheter available at the time?).
  • 1927 Moszkowicz proposes dextrose for injection.
  • Mariano (1949) suggested ”salicylate” as the substance. Aspirin?
  • In the 1940s, “sodium morrhuate” was recommended 5-10%. When this substance was used, painful thrombophlebitis occurred along the vein with a very severe chemical reaction, and although it was such a rapid and severe reaction, it was recanalized in a shorter time. Reopened.
  • Today, “Lauromacrogol” is used for sclerotherapy.
  • In a short time, the use of urinary catheter was abandoned.
  • There is a lesson to be learned from history at this point. With this sclerosing agent injection, the VSM was attached to the groin as well.
  • In summary, if there is an underlying venous insufficiency. Without correcting it, the success of the foam treatment will decrease.
  • First of all, it would be more appropriate to perform sclerotherapy after the underlying insufficiency ligation and/or endovenous ablation is resolved.
  • We should always keep this in mind.
