CHOCOLATE CYSTISTS IS ENDOMETRIOSIS A PERSONAL DISEASE?
Endometriosis is a chronic disease that can be observed in 1 out of every 10 women in the society, can involve various tissues in women, and reduces life standards. This disease can spread, just like tumors, and so its stage increases. But let’s explain this issue, this disease is not a cancer. Chocolate cysts are seen in the ovaries in the advanced stages of this disease. It can be unilateral or bilateral.
WHAT ARE THE SYMPTOMS OF CHOCOLATE CYST?
As it will not cause any problems, it can cause severe inguinal pain, nausea, abdominal pain, severe pain during menstrual cycles and infertility. Besides pain, it is also counted among the causes of infertility. Especially large cysts can turn the ovary around and disrupt blood flow or burst, creating an emergency.
When is it necessary to treat these cysts, which systems are used in the treatment?
Chocolate cysts, especially when they are unilateral and very painful, can be decided for surgery by looking at their size. If there is a young patient, a decent egg reserve and a large size, an operation plan can be made. We usually recommend laparoscopy, that is, closed surgery. It is very valuable to look at the ovarian reserve while making this decision. If the reserve is low, which means that the AMH value is <1ng/ml, the decision to operate should be reconsidered. Because even in the most skillful hands, there is a decrease in postoperative reserve. This carries the risk of early menopause. Surgery is risky, especially in the presence of large 2-sided cysts and if there is previous surgery.
HOW TO TREAT CHOCOLATE CYST?
These cysts can be treated with hormones for 3-6 months in some cases. Cysts can be suppressed with hormones if a surgical plan is not made, especially in women who describe severe and persistent pain during menstrual periods.
DO WOMEN WITH CHOCOLATE CYST HAVE CHILDREN?
What matters is the ovarian reserve and the condition of the tubes. A woman with sufficient reserves and open tubes can become pregnant ex officio. If there is an operation plan, ex officio pregnancy can be observed in the first year, especially in advanced stages.
IVF is a very effective form of treatment, but it is not wrong to recommend it to everyone all the time. The following 4 issues are very important for making an IVF decision: Women’s age, ovarian reserve, marriage deadline, sperm characteristics, etc. should be taken into consideration.
