Metabolic syndrome is a fatal disease that increases in frequency with changes in our dietary habits and inactivity, causes significant problems if left untreated, progresses with abdominal fat in which insulin resistance is the center, high sugar level, increase in blood fat, high blood pressure and increase in belly circumference. . According to the results of the Turkey Metabolic Syndrome Survey conducted in our country in 2004, the prevalence of metabolic syndrome in adults aged 20 years and over was found to be 35% (41.1% in women, 28.8% in men). In the Metabolic Syndrome Society Turkey Health Study (PURE TURKEY) conducted in 2010, 4057 individuals were included in the study, and waist circumference > 94 cm for men and > 80 cm for women was taken as criteria; The prevalence of metabolic syndrome was 43.5% in women and 41.4% in men. In the same study, the frequency of metabolic syndrome was found to be 57.7% in individuals aged 60-64 years, with the increase in metabolic syndrome frequency as age increases. Another feature of this study is that 63.6% of women and 34.5% of men were obese. Although various associations have put forward different criteria for the diagnosis of the disease, the most frequently used one is the National Cholesterol Education Program Adult Treatment Panel (NCEP-ATP III) revised in 2005, in adults, abdominal obesity (>102 cm in men,>88 cm in women), hypertriglyceridemia (>150 mg/day). dl), low HDL (<40 mg/dl in men, <50 mg/dl in women), hypertension (blood pressure >130-85 mm-Hg), hyperglycemia (fasting blood glucose >110 mg/dl). Studies have reported that the presence of three of these five criteria is sufficient for the diagnosis of metabolic syndrome
THERAPY APPROACHES IN METABOLIC SYNDROME
The primary approach in metabolic syndrome, which is a disease caused by environmental factors other than genetic features, should be the regulation of lifestyle. . The aim is to protect the individual from diabetes and heart disease. Weight loss, provided by a suitable nutrition and exercise program, has a corrective effect on sugar development and occlusion of the heart vessels.
LIFESTYLE CHANGES
In the treatment of metabolic syndrome, prevention of abdominal obesity seems to be a primary solution. For this, a lifestyle that will provide and maintain a 7-10% decrease in total body weight in 6-12 months; It is reported that it can be achieved by restricting calorie intake, increasing physical activity and motivating people in this regard. With an appropriate exercise program, insulin sensitivity is increased by increasing energy consumption gradually, so that both the risk of cardiovascular events and the development of metabolic syndrome can be reduced. Current clinical evidence shows that even approaches that provide 150 minutes of weekly physical activity and only a 5-7% reduction in body weight are sufficient to prevent metabolic syndrome; It is that it improves blood fats, insulin resistance and normalizes blood pressure. In general, for people with metabolic syndrome; Dietary models that are limited in saturated fats and cholesterol, rich in complex carbohydrates, consumption of plenty of fruits and vegetables, and salt restriction in those with hypertension are recommended. The traditional Mediterranean diet is one of the most important treatment options for the prevention of coronary heart disease and metabolic syndrome. It is argued that olive oil, which is an important component of the Mediterranean diet, also has an effect on lowering blood pressure. Similarly, there are epidemiological studies showing that increasing the consumption of foods such as fish, vegetables and fruits rich in omega-3 fatty acids and antioxidants, which are another important component of the Mediterranean diet, dried legumes, unpurified grains, reduces the risk of coronary diseases and the risk of death. Slowly digested low glycemic index diets may also have beneficial effects on lipid metabolism. Foods with a low glycemic index can reduce insulin resistance and improve metabolic syndrome. In the DPP study, the risk of diabetes decreased by 58% with lifestyle changes. The risk of diabetes was reduced by 31% in those using metformin. High blood pressure can also return to normal with diet therapy and weight loss. In all studies
, lifestyle regulation is the primary and most effective approach in the prevention and treatment of metabolic syndrome.
